Update HMS hodo parameters for 4/4 trigger

The 4/4 trigger is about 23ns different than the 3/4 trigger
So need different hhodo_tdc_offset parameters
Updated the hhodo_4of4trigger.param to have better hhodo_tdc_offset parameters

Modify DEF-files to use new trigger apparatus leaf structure. Rename …

The old map file was missing one wire per plane. The new file also
does not have these 'lone wires', where there is only one wire
listed in a separate slot.

Updated SHMS DC parameters.

- Changed `pdc_alpha_angle` to be consistant with my understanding of this angle.
- Corrected `pdc_nrwire` to actual wire number count.
- Updated `pdc_central_wire` to what Deb sent me.

Update Parameter files

Modified standard.kinmatics to set ptheta_lab and ppcentral
Modified pcal.param to eliminate stuff left over from when used in standalone
Modified shmsflags.param to eliminate ppcentral and ptheta_lab which should be set in standard.kinematics
Modified phodo.param to have better time offsets and set cosmicflag.

Update hhodo.param time offsets

Delete duplicates in hhodoana.def
Add beta and fptime histograms to hhodoana.def

First pass through SHMS DC parameters.

- Fixed some parameters.
- Reformated files.
- Moved some parameters from `pcana.param`.
- Reformated `README.md` in order for pandoc to correctly do nested lists.
